Through our partnership with Women’s & Men’s Health Services of the Coastal Bend, Inc., we are making it easy to get your required medical clearance.

📝 Who Needs One?
MANDATORY: All incoming 7th, 9th, and 11th grade students.
IF NEEDED: Incoming 8th, 10th, and 12th grade students.

You MUST bring a completed Medical History Form and Consent Form with you.
Forms will be handed out to students at school—keep an eye out for them!
No forms = No physical. Make sure they are signed and ready to go!
